Middelburg, Mpumalanga – November 28, 2025 – A tragic head-on collision between two bakkies on the N4 highway near Middelburg Dam has resulted in the deaths of three individuals, with several others sustaining injuries in the early morning incident.
The crash occurred at approximately 11:30 on Friday, involving two light delivery vehicles traveling in opposite directions. According to preliminary reports from authorities, the driver of one bakkie, heading from the direction of Mbombela toward Emalahleni, lost control of the vehicle. The bakkie veered into the oncoming lane, where it collided directly with the second bakkie approaching from the Emalahleni side.
Emergency services responded swiftly to the scene, where paramedics confirmed three fatalities at the site. The deceased include the male driver of the errant bakkie and two of his passengers: a young boy and a female adult. Among the survivors from both vehicles, two young girls were transported to a nearby hospital with serious injuries. Additionally, the driver of the second bakkie, along with two passengers—a female and another young boy—sustained minor injuries and were also rushed to medical facilities for treatment.
The N4, a major route connecting key economic hubs in Mpumalanga province, was the site of this devastating accident, highlighting ongoing concerns over road safety in high-traffic areas. Officials from the Mpumalanga Department of Community Safety, Security and Liaison have launched a full investigation to determine the exact circumstances, including potential factors such as vehicle condition, road surface, or driver impairment. In a statement addressing the incident, the Mpumalanga MEC for Community Safety, Security and Liaison extended condolences to the families of those who lost their lives. “Our hearts go out to the bereaved families during this difficult time,” the MEC said. “We also wish a speedy recovery to the injured parties and urge all road users to remain vigilant.”
This collision adds to a series of serious accidents reported on the N4 corridor this year, underscoring the need for heightened awareness among drivers.
